Read this carefully before you start. These instructions include essential information for your application.

This form is used to apply for STCW certification only For domestic near coastal certificates, see Domestic Certificates of Competency (COCs)

THIS IS AN INTERACTIVE FORM. YOU MUST SELECT THE CORRECT APPLICATION TYPE AND CERTIFICATE.

The form will generate a checklist for your application based on what you select. Complete all sections on the form and select ‘Download pdf’ once you have done so. Provide all the supporting documentation listed on the checklist when you lodge your application.

Australian residents:

  • You must lodge your application in person at a participating Australia Post Outlet,
  • Take your form and all supporting documents indicated on the checklist to Australia Post,
  • Sign the form in front of the Australia Post service person.

Overseas residents:

If you do not provide all the required documents, your application will be considered incomplete.

Lodgement of this form DOES NOT:

  • represent acceptance of your application as complete;
  • automatically entitle you to receive the certificate or endorsement you have applied for; or
  • indicate acceptance of any provided sea service.

Your application still needs to be assessed by a certification officer and decided by AMSA. We will decide your application within 28 business days from the time we receive a completed application. This includes all information needed for a decision.

Proof of identity

Proof of identity is required when applying for any certificate or seafarer’s service book. Australian residents must take original documents to Australia Post to prove your identity.

YOU MUST CHOOSE ONE OF THESE FIVE IDENTITY DOCUMENT OPTIONS:

Option 1

  • Your current passport

Option 2

  • Your current Australian driver’s licence, and
  • Your full Australian birth certificate or your Australian citizenship certificate.

Option 3

  • A photographic identification document issued by an Australian Commonwealth, state or territory government agency, and
  • Your current Australian Medicare card, and
  • Your full Australian birth certificate or your Australian citizenship certificate.

Option 4

  • Your current Australian Medicare card, and
  • A current account card from a bank that shows your signature, and
  • A proof-of-age card or student ID card that shows your photograph, and
  • Your full Australian birth certificate or your Australian citizenship certificate.

Note: If the name on the identification documents (passport, driver’s licence, birth certificate, or citizenship certificate, etc) does not match the name on your application form, you must provide change of name evidence.

Option 5 – Aboriginal and Torres Strait Islander people

Provide a copy of your birth certificate. Aboriginal and Torres Strait Islander people born earlier than 1969 who do not have a birth certificate may be recorded in the Aboriginal Population Register available from the Northern Territory Registry of Births, Deaths and Marriages. We will accept this document as equivalent to a birth certificate. If you cannot provide proof of identity as outlined above, a completed Identity declaration for Aboriginal and Torres Strait Islander person Form 162 will be accepted as being equivalent to option 1 (current passport).

Application Type (select the type of application you are applying for)

CHOOSING THE INCORRECT APPLICATION TYPE MAY DELAY THE ASSESSMENT OF YOUR APPLICATION.

  • You must complete a sea service assessment prior to applying for the initial issue of either a certificate of competency or certificate of recognition
  • Separate sea service assessment applications are NOT necessary for Ratings
  • Select this for the initial issue of a certificate of safety training, GMDSS certificate, endorsements, certificate of proficiency (ratings and marine cook), and certificate of competency
  • For Deck and Engineer Officer Certificates of Competency, a positive, valid AMSA sea service assessment is required. You must also have passed an AMSA oral exam and include evidence of these with the application
  • Only select this if your existing certificate is lost, stolen, or damaged, or if you require a change of personal details i.e. name change
  • Only issued to seafarers from countries with whom AMSA has a MOU
  • Certificate of recognition applicants must hold a current, positive AMSA sea service assessment and have completed an oral exam
  • Applicants must have Australian working rights
  • Select this for revalidation of a certificate of safety training, GMDSS certificate, certificate of proficiency (ratings and marine cook), certificate of competency, and endorsements (other than ECDIS)
  • You must hold current AMSA-issued certification
Checklist for supporting documents

A checklist must be generated with this form.. DO NOT PROCEED WITH AN APPLICATION IF YOU DO NOT HAVE A CHECKLIST GENERATED WITH THE AMSA 419 FORM.

You must provide copies of all supporting documents to the Australia Post service officer. DO NOT provide original documents. AMSA accepts no responsibility for lost originals.

Sea Service Assessments

  • Provide entire copies of any STCW certifications needed.
  • Sea service letters must be signed, dated and stamped. A template can be found here.

Overseas applicants must:

  • send only certified true copies of documents,
  • scan and send a passport photo with your application,
  • sign in the specimen boxes on the last page of the AMSA419 form, and
  • send your scanned form and certified true copies to STCW.applications@amsa.gov.au

Important Note

Applications WILL NOT BE ASSESSED until all documentation on the checklist is received. We will contact you if your application is incomplete or we require additional information. Any fee you have paid will be withheld as outlined in our incomplete application policy.

Payment

Select ‘Download pdf’ at the top right of this form to download your completed application, then lodge your application with supporting documentation and pay at your nearest participating Australia Post Outlet. A tax invoice for your fee payment will be issued to you when your application has been processed.


AMSA privacy policy

The Australian Maritime Safety Authority (AMSA) is collecting the information on this form to determine if you are eligible for the issue of a:

  • Certificate of competency, or certificate of recognition as a master, deck or engineer officer, or GMDSS radio operator’s certificate; or
  • Certificate of proficiency, rating, or safety training.

The collection of the information is required, authorised or directly related to the Navigation Act 2012 and the Marine Orders made under it. It will be used for the purposes related to the Act and Marine Orders and will be treated in accordance with the Australian Privacy Principles. AMSA may give some or all of this information, including information concerning the validity, cancellation, imposition of restrictions or suspension of certificates to overseas marine administrators, ship owners, operators, agents, the Australian Communications and Media Authority and law enforcement agencies as required by law. Failure to provide the information may result in the transaction not being processed.

Proof of identity

Proof of identity documents must be presented to Australia Post at the time of lodging your application. These MUST be original documents.

Your photograph will be taken upon lodgement of your application at Australia Post at no extra cost to you.

Overseas applicants should include photos as per specifications below:

Include two photographs of yourself, taken within the previous six months. Attach one below and include a second with this application. Both photographs must be certified on the reverse side and have dimensions of 45 mm x 35 mm (not including a border), as indicated below.

The certifying officer must state: "This photograph is a true likeness of (your full name)." and include the date of certification and the full name, signature and title of the certifying officer.
