When to use this form

Use this form to apply for AMSA accreditation in marine surveying:

  • for initial application; or
  • to add other category(s) to an existing AMSA marine surveyor accreditation

General criteria for the issuance of marine surveyor accreditation

Before submitting the application for initial or additional accreditation, the applicant must ensure that they can demonstrate that they meet the criteria in section 25 of the National Law Regulation. If you are unsure, please contact the Accreditation team at dcvsurvey@amsa.gov.au to discuss further.

How to complete the form

Online

Type directly into the boxes and on completion, select “download pdf”. The system will generate a prefilled form with a unique AMSA Transaction Reference Number (TRN) beginning with 160741xxxxxxxx located at the top right-hand corner of the form and on the Fee Advice located on the last page. The TRN number will link your application and payment. If you make an error re-complete the form, and a new TRN number will be generated. The form must be completed online because the fee is calculated based on the number of categories. If you are having trouble filling the form, please email dcvsurvey@amsa.gov.au for the attention of the Accreditation Team.

Print & sign

Please ensure that you sign strictly within the box provided using a black pen. If your signature is too large to fit in the box, you may submit a separate sheet of paper with your signature. The signature box can be enlarged by clicking and dragging the bottom right-hand corner of the box before printing. Please print all pages including Fee Advice.

What do you need to provide

You will need to provide these supporting documents:

  • Current curriculum vitae
  • Certified copies of qualifications relevant to the category(s) applied for
  • Evidence of continued professional development (CPD) relating to vessel building, design, engineering, or vessel survey
  • Current professional association membership relevant to vessel building, design, engineering, or vessel survey
  • Evidence that vessel surveys will be conducted either:
    • under an accredited ISO 9001:2008 compliant Quality Management System; or
    • an equivalent management system; or
    • in accordance with Part 1 of the - Marine Surveyors Accreditation Guidance Manual (e.g., by Letter of Undertaking)
  • Current professional indemnity insurance policy certificate of entry for at least AUD$1,000,000; or
    • Quote for the insurance; or
    • If the policy is issued outside of Australia, a letter from the insurance broker for the policy confirming the policy
  • Certified copies of identity document (see section E of this form for specific requirements)
  • Current photo (see section F of this for photo requirements)
  • Details of two (2) referees who are your peers in marine surveying and related industries
  • Relevant fee

The fee payable is an assessment fee determined by the number of categories applied for.

If a decision is made to refuse your application, your fee will not be refunded.
