As per Marine Safety (Domestic Commercial Vessel) National Law Regulation 2013 you are required to submit your application AMSA804, and pay the fee no less than three (3) months prior to the expiry of your accreditation.

If you are unable to submit your application three months before your accreditation expires, please contact the accreditation team to discuss the options via email dcvsurvey@amsa.gov.au.

If you do not wish to renew your accreditation, please notify us by reply email dcvsurvey@amsa.gov.au.

When To Use This Form

Use this forms to apply to:
  • Renew your marine surveying accreditation

Instructions for Completing the Online Form

Type directly into the boxes and once completed, select “Download PDF”. The system will generate a pre-filled form with a unique Australian Maritime Safety Authority Transaction Reference Number (TRN) beginning with 160804xxxxxxxx located at the top right hand corner of the form and on the Fee Advice on the last page. This TRN will link your application and payment. If you make an error simply re-complete the form, and a new TRN number will be generated.

Signature Instructions

Please ensure that you sign strictly within the box provided using a black pen. If your signature is too large to fit in the box, you may submit a separate sheet of paper with your signature. The signature box can be enlarged by clicking and dragging the bottom right hand corner of the box before printing.
Please email all pages including the Fee Advice.

What You Need To Provide

You will need supporting documents evidencing:

  • Current professional association membership
  • Continued professional development (CPD) completed in the last five (5) years
  • Photograph

The final page of your application contains the Fee Advice. Use this to make payment prior to lodging of your application.

Lodging Your Application

You must email documents at the same time as the AMSA804 form. Applications missing required documents will not be processed and will be returned to the applicant.

C. Supporting Documents for Professional Association Membership

  • AIMS stamp showing membership level
  • RINA letter confirming class of membership
  • IIMS membership card

  • A written statement from your professional association
    A tax invoice alone is insufficient, as it does not provide proof of payment, membership details, or the membership expiry date.
